The patient will show up to his regular visit or appointment with his physician, and then will prescribe his regular medication. After that, the physician will refer the patient to the diabetic pharmacy clinic service for his medication .The patient will take an appointment for the pharmacy clinic by scheduling an appropriate timing for the patient and the pharmacist; the scheduling will be done by medical information management or diabetes register. The diabetes register is used to recall patients at specific intervals and for annual review. For non-attenders a choice should be made within the team to agree the process for follow-up, e.g. send total of 3 appointment dates, and no attendance, document and flag up on repeat prescription. Patients who should be seen in the clinic include, hospital discharge (newly diagnosed patients with diabetes, patients discharged from hospital clinics), self-referral, referral by other health care professional, and annual review. A standard letter should be sent to the patient at least two weeks before the clinic accompanied by a blood form, urine sample and the dietary assessment form. Organize the following tests, body mass index blood pressure, HbA1, and lipids. Refer the patient as to the appropriate services for advice on smoking cessation, alcohol problems, nutrition advice and exercise. Specialist referral by the GP may be required for aspects of metabolic control and diabetic complications. On the other hand, protocol for annual review include, arranging the following tests: body mass index, blood pressure ,urine microalbumin,HbA1 ,lipids, and LFT's if on a glitazone.
