On 4/26/2016 SC met with Pa and completed a home visit. The Pa appears to be in pain and was leaping. The SC asked the Pa if she ok and she said yes but she experiencing a lot of and is always in pain. The SC asked the Pa if she talks with her doctors about her pain and she said yes. Pa reported that she lost saw her PCP on 4/20/2016. But reported that she has not seen her Rheumatologist in a long time because the lost time she was there he had refused to write her pain and instead give her injections. The Pa reported that the injection did worked but she was offended by the doctors taking away her Rx pain because of past history of substance abuse. The Pa reported that it’s from her lupus. The Pa then pointed to her right elbow/arm and showed the SC the scares on her arm from a recent lupus flare-up. The Pa reported she is recovery well from it. She has a few appointment coming up but is not sure if she going to keep those appointment because of how she is feeling now. The Pa was encouraged to keep her appts if possible to ensure proper and constant care. The SC asked about the Pa’s use of medical service in the lost several months. No ER, hospitalization, change in medication or health status and no falls was reported. The Pa was dressed in white t-shirt and pink PJ’s bottom with hearts and the foot rolled up past her ankles. She appeared clean and properly groomed. Pa’s home was clean, well-organized, and free of clutter and hazard free. The Pa reported that all utilities are in working order. The SC reviewed the Pa’s service. Pa reports that she is receiving the services in the type, scope, amount, frequency and duration as specified in the ISP. Pa received Pas 7 days 4:30 hours in split shift of 8-12:30 pm PAS assists with bathing, dressing, grooming, laundry, shopping, light housekeeping, meals prep and other errand. Pa also has ERS which she uses to call for help in the event of an
