Case Analysis: Depression and Suicide Ideation with a Suicide Plan This is a case analysis of client, Harold, referred to Greenbrier Behavioral Health Center, an intensive outpatient program (IOP), from Greenbrier Behavioral Health Hospital. The client is a 43 year old, retired, white male, living in Slidell, Louisiana. The highest level of education completed is a GED. He is married with two children, ages 8 and 12. In 2013 he retired with more than 20 years on the police force. Harold is overweight, wearing sweatpants and a t-shirt, appears clean and groomed. He is sitting, calm and quiet with arms resting on his legs. Harold grew up in Louisiana with his mother, older brother and older sister. At that same time, Harold’s sister was in a car accident, left bedridden, and permanently in need of care. Six months later he and his wife decided to spice up their relationship and invited a man to participate in their sexual life. She confessed two weeks ago she had feelings for this man and had been seeing him without telling Harold. Harold became angry, started drinking again, stopped self-care, became withdrawn and sad, discontinued leisure activities, and began having suicidal ideation. Harold took his gun to his backyard intending to shoot himself in the head when his wife and children arrived home. His wife saw him in the backyard and intervened. Harold’s wife took him to Greenbrier Hospital where he admitted himself. Harold received a diagnosis of depression with a referral to Greenbrier …show more content…
The groups offer open enrollment. Since the insurance company agreed to thirty days of treatment for Harold, he is placed in the higher functioning therapy group consisting of members with diagnosis of depression, anxiety, bi-polar, complicated grief and those with suicidal ideation or attempts. He will participate in the group five days a week for the first two weeks and begin decreasing weekly sessions to three days a week with progression. Harold is set up to visit with a nurse and psychiatrist weekly. The psychiatrist prescribed Effexor for his depressive symptoms. Harold’s treatment goals are to decrease depressive symptoms, identify stressors in relationships, decrease thoughts of suicide, and refrain from alcohol use. Greenbrier staff will monitor Harold for expressed suicidal thoughts and continue to evaluate the treatment plan and
