Hello Vitaliy,
I like how you start off getting your supplies and also your pen light; however; you did not introduce yourself before asking question. I know you probably would if you was actually doing an assessment but though to mention this. You did great ion asking about the use of tobacco, alcohol history and illness for the dental record. I like how detailed you were when you were assessing for the proper color of the teeth, tongue, gum and if there were any redness, swelling using your pen light. You also checked for lesion and bleeding or cuts on the tongue. You have a great information and only think I notice was the greeting part which is important. 
