12/23/15 Progress Report noted that the patient was seen today for a follow-up. She remains entirely compliant and cooperative with all the treatment recommendations. She continues to be socially isolated, but she is more active at this point in time than she has been in the past. It was recommended that she continue under psychotherapeutic care of Nelly Saucedo, LCSW. She continues on Xanax 0.25 mg and Melatonin. Her next psychiatric appointment will be in 6 weeks. She states that she has had an excellent reputation. The first 17 years, she was a LH/PH tutor, but 10 years ago her Director moved her to the office. She got a position change and became a Special Ed. Enrollment liaison. She was told in June that her position was abolishing and she was sent back to LH/PH position. She felt humiliated and felt if her work did not matter. She felt unimportant. She is unable to sleep and she worries all the time about driving into horrible neighborhoods. She has no support. She is travelling to three schools, over 200 miles a month. She feels too old to interact with the children and she is becoming more and more depressed. She endorses symptoms of Major Depression, Generalized Anxiety and Social Phobia. Since her initial treatment, she has shown some periods of improved functioning, marked by depression and anxiety, as well as increased positive cognitions. However, the continued difficulty she has had in adjusting to not being in her work environment has affected her ability to maintain the gains obtained. She continues to have feelings of worthlessness, of not mattering, of being irrelevant. She stated that anxiety and frustration have become her new normal. The client’s anxiety and depression have reached a state of permanent and stationary. Recommendation: psychotherapeutic
