1. Introduction- What went wrong?
Delegation is a process of decision-making based on assessing, planning, communicating, supervising, and evaluating the care provided (Marquis & Huston, 2015). The nurse is accountable for the decision to delegate the tasks/activities and responsible for ensuring the nursing care is performed correctly according to a standard of practice and facility policy (Marquis & Huston, 2015). In this case scenario, violation of delegation was presented. It illustrated inappropriate actions taking by a nurse when she delegated nursing tasks to the UAP. The nurse cannot delegate the nursing process and the tasks that require nursing judgment to the UAP (Marquis & Huston, 2015). In this case, the primary nurse should assess the patient first because she must use professional judgment to determine whether the nursing task can be delegated in this situation. Complex and unpredictable nursing care needed by the patient would require the care to be provided by a nurse (Marquis & Huston, 2015). The task must be properly and safely performed by the unlicensed person involved without jeopardizing the patient’s safety. Communication is the second principle that also was violated in this case scenario. Poor communication was presented throughout the video. The primary nurse was focusing on giving report while LPN was focusing on passing medications. Effective delegation requires on-going communication between the nurse and the UAP. The nurse is responsible for providing clear and instructions and guidance regarding the tasks/activities being delegated (Marquis & Huston, 2015). In this situation, the nurse didn’t provide concise instructions for performing the task. The primary nurse should directly communicate with the LPN and specifically tell her to check the chart and perform five rights before giving
