DHDx is the identification of an existing or potential oral health problem based on clinical diagnosis involving color, shape, location and history of the lesion. In other words, it is a report of what the dental hygienist sees as abnormal in the patient's mouth. Basically, the Dental Hygiene Diagnosis is the bridge between the evaluation phase and the treatment plan.
