Date Worker Observation Report

On this date worker visited Ms. Brooke Henry at Pine Place, for the purpose of monitoring her placement. During visit worker learned, Brooke had been standing in a corner. When questioned about this Brooke stated she was putting herself in the corner because she felt like she was being punished. When questioned punished for what Brooke could not answer. During interview, Brooke stated she did not like it at Pine Place. Worker explained the placement was only temporary until other arrangements. During interview, Brooke stated she the employees force her to remain in her room all day. Worker has spoken to employees and Ms. Tyree all stated they have to force Brooke out of her room when it is time to eat.
