The project focused on admission over a span of 3 months which might not be representative of all admissions.
The project employed a qualitative research design.
The population included 45 patients who did not have conditions that predispose them to DVT.
A purposive sample of 16 patients/charts was selected from the 45 available patients.
The project focused on a time that have seen a high rate of admissions within Hillcrest Convalescent center.
The period involved patients who were admitted from the beginning of November 2015 until the end of January 2016.
The study focused on admissions over a span of 3 months.
The project involved chart reviews of patients admitted to Hillcrest within three month.
The process involved determining patients admitted following a knee or a hip surgery and physically reviewing charts to determine if a patients had received a diagnosis of DVT while staying at Hillcrest Convalescent Center.
The process also involved reviewing charts to determine the characteristics of patients who have received DVT and those who did not receive

I have purposively selected a sample of 16 after excluding patients with predisposing factor of DVT. 2 patients received the diagnosis of DVT which may not seem to be significant. However its implications on the facility and the patients may have been tremendous

Both patients who have received DVT were obese, aged over 60 years old, and did not adhere to the DVT protocol even when it was recommended by the provider.

The patients who did not receive DVT were aged between 42 and 81 years old and most were overweight. However, they followed the DVT protocol as directed by the provider.
This project constituted a learning experience as research projects require a great deal of planning, data collection, and data analysis.
The DVT prevention protocol adopted by the facility is effective at controlling the number of DVT cases as the percentage of people who have developed DVT remained very low among people admitted with a knee or a hip surgery.
DVT prevention protocol at Hillcrest Convalescent Center is effective at keeping DVT cases

Another assessment the nurse monitors for is the formation of thrombosis or clots. “Because patients undergoing prostatectomy have a high incidence of DVT and pulmonary embolism, the provider may prescribe prophylactic low-dose heparin therapy” (Pellico, 2013, p. 963). Heparin helps to decrease the formation of clots, which can have a dangerous outcome if not caught early. While a patient is on heparin therapy, the nurse should cautiously monitor the client for bleeding (Pellico, 2013). Anxiety related to health status is a nursing diagnosis that would be appropriate for a client who is about to receive treatment.…

Survey suggests that VTE prophylactics are poorly implemented within the Australian hospital, therefore putting more patients at risk. Providing evidence-based practice to improve on this current issue should raise awareness. Nurse plays a vital role in assessing if VTE prophylaxis is suitable and improving this by ensuring that the patients are getting the right treatment in prevention of VTE. By informing the patient on how VTE can be prevented and the options available, they will be more aware of how the benefit will outweigh the risk in the long term. With good clinical care and appropriate timing, methods of pharmacological and mechanical prophylaxis will promote the prevention of VTE.…

Preventing VTE in the Hospital Setting VTE stands for vein thrombosis embolism. Thrombosis is a blood clot that forms in the vein and an embolism is anything that travels through the veins until it gets to a spot where is cannot pass through. Embolism then lodges its self into the vein and occludes blood flow to the rest of that area. “An embolism usually is a piece of blood clot that breaks off but it also can be fat, air, amniotic fluid, a tumor, or a foreign substance such as talc, iodine, cotton, or a tiny piece of catheter tube” (WebMD, 2016). There are several kinds of embolism if is in the deep vein, usually the leg, it would be referred to as deep vein thrombosis if the embolism breaks off and moves to the lungs it is considered to be a pulmonary embolism.…

Recently the patient was given an authorization to not use stairs. This authorization was given so that he would not cause further injury delaying his SRD. I am of the opinion that he could use stairs; however, preventative measures were thought to be necessary to prevent possibility of further injury prior to SRD, thus eliminating his capability of using stairs for bathing at time of discharge. Therefore, I feel that our use of no stairs now as a preventative or a prophylactic approach so that he will be able to use stairs once discharged back into civilian life.…

Deep vein thrombosis (DVT), occurs when a blood clot forms within a vein that's deep in your body. It typically forms in either the thigh or the lower leg. It mostly strikes people who spend hours sitting in one place, which is why it happens to airline passengers on long trips. However, commercial or owner operator truck drivers who spend hours on the road sitting in front of a steering wheel are also at risk of getting this condition. Sitting in the driver's seat for hours slows down your blood flow because there's no need for your heart to pump vigorously.…

One intervention that has been researched that nurses are able to provide to their patients to help with this type of condition is by equipping their lower extremities with graduated compression stockings. Taking an evidence-based approach to deep venous thrombosis research studies reveals the clinical practice guidelines are a valuable tool to improve clinical practice and patient outcome. Preventing deep vein thrombosis in the inpatient hospital setting is important to reduce the risk of fatal and non-fatal pulmonary embolism and post-thrombotic complications. Proximal deep venous thrombosis (DVT) can cause post-thrombotic syndrome and can develop into life-threatening pulmonary embolism (Restrepo, P. Jameson, D. & Carroll, D (2015). Therefore, the intervention to improve VTE prophylaxis is among the top ten patient safety strategies for the patient who has had recent surgery, immobility and/or multiple co-morbidities (Arabi, Y. Saud, K. (2015).…

This research study found that there were no deaths among the low risk patients during the three months follow up period. One hundred and fifteen (47.3%) patients treated as outpatients showed no deaths or adverse events such as bleeding. Even though low risk patients are candidates for outpatient care, this study’s outpatient group included 34 (29.6%) classified as high risk by PESI, and 54 (47.0%) classified as high risk by sPESI. From the outpatient group of 115 (47.3%), only one patient had a recurrent DVT within 30 days of follow up. The authors concluded that the two scores are accurately identifying low risk patients with acute PE who can be safely treated as outpatients (Erkens et al., 2011).…

    Acute Pulmonary Embolism

Acute pulmonary embolism (PE) is a common diagnosis throughout the world. In the US it affects approximately 1 in 1000 people and ranks as the 3rd most common cause of death in hospitalized patients. More than 80% of PEs are associated with DVTs in hospitalized patients with risk factors being recent trauma, surgery, immobility or obesity. While it can be entirely asymptomatic, nearly 25% of patients are thought to experience sudden death as their only manifestation of disease.1 Because of this breadth it can be easily missed, and so the true prevalence and manifestations of this condition are not well understood.…
