A medical necessary form I also known as a DM E information form is a form that is required to get medical equipment. The form is used to ensure that the patient needs this equipment and it's filled out in parts. Parts A and D I completed by the physician and parts B and C are completed by the supplier. Once the information is filled out a ticket is created and submitted to the insurance provider so that the provider can cover song and in some cases all of the cost in order for the patient to receive the medical equipment that they will need to ensure the best quality of life. This form is used to obtain durable medical equipment which includes wheelchairs, walkers, power chairs, lift mechanism's for recliner's, potty chairs, prosthetics, and
