Cypress Gardens Care Center Case Study

On Tuesday, February 21, 2017, at 5:00 a.m., the investigator arrived at the Cypress Gardens Care Center, located at 9025 Colorado Ave., Riverside, CA 92503 to conduct a scheduled AOE/COE investigation with four witnesses. Administrator, Mr. Lawrence Talabi's provided his r/s. However, the three witnesses did not show up due to a conflict in their work-schedule. During the interview, Mr. Talabi apologized requested to reschedule Ms. Anita Greenlee to Ms. Rosa Contreras interviews for Wednesday, 21, 2017. In the case of Mr. Charles Cornelius's statement, Mr. Talabi requested that Mr. Conrneilius be interviewed this afternoon when he reports for work. Mr. Talabi confirmed Ms. Engracia Alcala’s full-time Certified Nurse Assistant with the insured.
