Critique of Evidence
In review of, Continuous Positive Airway Pressure as Treatment for Systemic Hypertension in People With Obstructive Sleep Apnoea: Randomised Controlled Trial, (Duran-Cantolla et al. 2010), one strength included the study design; this was a randomized controlled trial. Other strengths consist of sample size, and the diagnosis of sleep apnea was conducted by the same method. In addition, the patients were newly diagnosed with hypertension, habitual snorers, and had not received any treatment other than conservative measures. In addition, obstructive sleep apnea affects 70% of those hypertensive patients that require multiple medications to reduce their blood pressure. Making changes in one’s therapy has the potential to reduce the risks of the aforementioned. The studies researched by the student show that there is a statistically significant decrease in blood pressure after implementation CPAP therapy. The student proposes a prospectus that examines CPAP therapy in hypertensive patients and compliance with prescribed use. The student will focus on ways to improve CPAP compliance to help augment the decrease in blood pressure over three months. The study will somewhat replicate Martinez-Garcia et al.’s Effect of CPAP on Blood Pressure in Patients With Obstructive Sleep Apnea and Resistant Hypertension (2013). The premise behind focusing on compliance is that some patients who would benefit significantly from CPAP therapy may be resistant to it because of the cumbersome and bulky devices.
