The pt was in lead III so I asked to see lead II, as respiration are seen in lead I and II. She stated that she had worked for two hours with out any success , she needs to prepare for report now and did not have time for questions. I pointed out that we were also getting ready for report and as it has been two hours would it be better for someone come down after report? She said that she only wanted to make sure that we were aware, we could come down when ever.

I said that the leads needed to be changed, there is nothing showing up on the screen She said that it wasn't looking like that a few minutes ago. I changed the leads back to lead III and said that I needed to see lead I or II as those were the repository leads. She said that she was in report. I said asked if it would be better for us to come back after report? She asked me if I was from Central Tele? I said yes. She asked me what my name was. Katie. She said that she needed both names. I am going to SRS you. I said Appleton. She stood up ;and asked what needed to be done. I said that new leads should be placed on the pt and preferably placed farther

I had set in mind the objectives that I wanted to accomplish for the day. One of the goals was to do a complete charting on one patient and my other goal was to work on time management. Charting in PACU is quite different than a regular floor because in PACU the charting has to be every five minutes times three when the patient arrives to the unit and then every fifteen minutes for about two hours until the patient is stable. When the patient is stable the vital sings and assessment are done every thirty minutes. As soon as the patient arrived to the floor, I was ready to connect my patient to the monitors and assess my patient.…

Also, I apply the six rights (patient, drug, dosage, route, frequency, day), and three checks every time I am going to administer a medication.
