Summary of Issue.
On October 20, David Newman contacted Human Resources to report an incident at the 25 Industrial Avenue site where human feces appeared to have been smeared on a toilet seat in one of the men’s bathrooms. Jeremy Varney, who is located at the site, reported the incident to David.

Jeremy stated that he became aware of the incident through his direct report Vincent Brown. He reported the incident to David Newman and the Directors at the site and David contacted Human Resources.
I asked Jeremey if he had any indication as to who may have committed the acts and he responded that there are strong personalities and individuals who are unhappy at the site but he could not imagine anyone committing such an act. …show more content…
The bathroom is located at the rear of the site near the copy machine. Vincent asked Ms. Vazquez whether she believed, the incident had occurred over night and she responded that, it appeared to have occurred earlier that morning.
I asked Vincent whether this was an isolated incident and he responded that there have been issues with bathroom cleanliness in the men’s bathroom in the past year. For example, feces smeared on a toilet seat and urine on the bathroom floor. He further added that approximately two weeks prior to the October 19 incident, Ms. Vazquez reported finding in the bathroom nearest the cafeteria, urine stains on the wall opposite the toilet
