On 06/28/2015 at approximately 0650 hours Mainentance Lenord Keenen and Payroll Clerk Erin Keenen observed Correctional Officer Alvin Barnes sleeping in the stationary vehicle posted near Zone one as they entered the gate into Central Arizona Correctional Facilty. According to the written statements made by Lenoard and Erin Keenen they observed Officer Barnes leaning on the door of the vehicle and his head was down. Mr. Keenen also obsereved Officer Barnes eyes closed at this time.

Gibson Ln Phoenix AZ 85009, Inmate Rubio, Daniel T391297 informed Officer Mossman B3777 that he was the victim of an alleged PREA violation while he was housed in Durango 9. Officer Mossman immediately removed Inmate Rubio from the housing unit for his safety and notified a supervisor.
