The reporting party (RP) stated on 2/13/17 resident John Gardner DOB: 7/05/54 was in the office displaying agitated behaviors. The RP stated the resident continue to repeat "home, home". A photo of the resident's home and the day program was provided to the resident to determine if he wanted to go home; John pointed to the day program. Subsequently he pulled on his shirt collar and pulled on his wrist pointing to the photo of the home. In addition to the two photos a picture of the van which transports the resident was provided to him and the question was asked if something occurred in the van; he continue to point to the home. John pointed to the photo of the home and began making hand gesture depicting his hand being pinned down and his hand
