Continuous Home Care: A Case Study

There are three types of program Continuous Home Care, Short Term Inpatient care, and Respite Care. Continuous Home Care is during a crisis to achieve palliative or management of acute symptoms. During this process Social Workers manage the resources of systems in which clients function. Short term Inpatient is for procedures; such as pain control, or acute, or chronic symptoms. At this moment the nurse will be needed as patients go through breakdowns. Also, this is the point consulting with clients regards the problem. The last program is Respite Care. Respite care is short term provided to individuals, while their family or person caring for them is away. (National Hospice and Palliative Care Organization, 2015)

Social Workers also provided each patient with their love to make sure they were doing okay. I’ve seen my Social Worker help feed the patients, and make sure their health was okay, and spend time with patient’s playing cards and coloring. Once again consulting with clients regarding problems was going on doing my visit. My Social Worker discuss with Mrs. Schiener niece, that she needs to stop getting out of bed by herself, because she can hurt herself. As a Social Worker we must get to know the patient, and their life, and their family as well as their situation of why they’re in hospice. My Social Worker provided service to each patient by discussing their social problems, and asking them how they’ve been doing during each visit. My Social Worker, also showed dignity and worth of the person by addressing each patient’s background with me when I first met them. The last value my Social Worker touched on was Importance of human relationship, which focused on the life and happiness of all people. She did whatever she could to keep each senior citizen joyful.
VOLUNTEER EXPERIENCE
I learned how different senior citizens are, because I assumed they were all evil. There were patients that were nice, and some mean, but majority were nice. All older people have different personalities, which Mr. Rowe kept me laughing. He continued to make different jokes every time I saw him. On the other hand, Mrs. Schiener was very pushy and mean. She made rude commented every time I made conversation with her.
The Strength to become a better Social Worker are providing patients with love and care, helping senior citizens with their needs and wants, and providing senior citizen with words of wisdom from God, and provide different activates for senior citizens. On the other hand, my weakness is feeding patients, making more communication with the patients, and encouraging them to do things for themselves, because the senior citizens probably won’t listen to
