Comprehensive Continuum Of Care

Chronic illnesses are only dealt with in the medical aspect. Physicians typically overlook the social aspect of these illnesses. However, how the patient functions in their day-to-day lives with a chronic illness is just as important as treating the illness. With any chronic illness, continuum of care is important. This would ensure that the patient is receiving the proper care in regards to the chronic illness and any other illness that may occur. It is important to ensure that the patient’s care is continuous once they leave the hospital or physician’s care. To ensure this, one must evaluate all the psychosocial factors and interventions involved in continuing the care of the patient.

When a patient leaves the hospital, care must continue in order to ensure the best prognosis for them. In many cases, there are services offered for the patient that will help make the transition easier for them. With this, several psychosocial factors come into play as well as the interventions to help the patient adjust to their new life. Social workers use these interventions and psychosocial factors to create a discharge plan for the patient.
