Goal 1: Justice will improve his interpersonal functioning and work toward abstinence.
• Justice’s mother was observed sweating and seemed sick.
• Justice’s mother informed the QP she was feeling dizzy because she took her insulin on an empty stomach.
• Justice’s mother reported she is a diabetic and has high blood pressure, plus she does not take her medication for any of medical conditions unless her sugar and blood pressure are high then she does not take her psychotropic medication.
• Justice’s mother stated, “I don’t know why I am not doing right and I am not eating like I am supposed too.”
• Justice’s mother agreed to set her appointment and follow up with the QP to let her know when they are scheduled.

• Justice’s mother informed the medication manager Justice is not out of his medication; however, she will take the prescriptions and dropped them off at the pharmacy, so he will have the refills.
• The receptionist scheduled Justice’s next medication management appointment for June 1st at 1:30pm.
• Justice’s mother listened as the QP shared that she found out about the last time Justice was at Carolina Smiles Dentistry.
• Justice’s mother stated she did not want to go to another place, but realizes Justice will have to be seen as a new patient since it has been so
