It is incredible that these errors occur, that mistakes like this can be made and get past the prescribing doctor, pharmacist, resident, fellow (?) on call physician and nurse! Although I’ve heard of these errors occurring before it’s always disheartening to know that so many eyes can overlook a medication error. Again, if you’re in healthcare to take care of people and your community then I think you are more likely to look up a medication before giving it to make sure it’s appropriate, helping to avoid such an error. In my opinion, medical staff that make medication errors such as giving the wrong dose or administering a patient someone else’s medications should lose their privilege to administer medication and their license temporarily or permanently.
