Community Patient Resource Group Interview Lab Assignment I interviewed a patient using a template by Fruth (2014), who was a 65 years old male Mike Hughes a Christian Baptist by religion who used English as his first language. He is a retired veteran by profession who worked for every 6 months while at military. On observation, the patient entered the room with a standard cane. The speech was not affected but there was evident drooping of lips on the left side. The patient’s chief complaint was weakness and reduced sensations in the left leg and left arm since June 2005 when he suffered from stroke. He received a day pass on occasion of 4th July to visit his family. However, when the staff were transferring him back from the ambulance he fell from his wheelchair. As reported by the patient this incident regressed the patient’s condition, i.e., the weakness increased on the left side. Since then, the patient is coming for physical therapy. In the past, the patient had an episode of heart attack on March 31 2004 for which he was operated and stents were placed in his heart. The patient also had C6- C7 nerve root impingement and underwent titanium cage neck repair in March 18th 2015. Patient was adopted so had little information about biological parents. His house has 1 staircase. He reports of using stairs at least once a day. The patient has a private bath at the same floor as his room. The flooring of the house is hard wood. The patient lives with his wife and drives a car by himself as a means of transport. Currently, the patient works as a helper for senior living homes. He visits one 70 years dementia patient and accompanies him for an hour or two. The patient lives in a house which has 2 acres of lawn. He loves to maintain the lawn by himself. He works on his lawn 4 hours at times. He also loves to fish and hunt as part of recreation. Currently, the patient participates in helping seniors. He drives everyday by himself. He loves to spend time with his granddaughters. He visits community hospital every week where he takes physical therapy. When asked about patient’s perception about his current health, the patient mentioned that he has improved 75% as compared to his past health status. Socially, Mike is an occasional drinker. He quit smoking since 2005 however he was a smoker for 40 years. He started smoking when he was 15 years old. He loves to drink iced tea and coke zero several times a day. He drinks one cup of coffee every morning. The short term goals for Mike will be:
1. The patient will be able to button his shirt and eat without difficulty initially with supervision in front of therapist and caregiver for at least 5 times a day for 3 days
