Community diagnosis The significant community diagnosis based on the assessment of the HUD senior housing is risk for injury and harm among HUD tenants as evidenced by tenants verbalizing insufficient street lights and construction of M-1 rail project at Woodard and () intersection. These are all safety concerns that can cause harm to the residents. The tenants verbalize that most of the time they take buses and walk down the street from their apartment to get grocery or go to their doctors’ appointments. However, since the construction started it is difficult to get around; especially for the tenant that uses a cane or wheelchair.
In addition, there are other risks that were identified during the interview. They are but not limited risk for depression, risk for sleeping disorder, and risk for substance abuse related to living in a stressful environment and lack of physical mobility.

For instance, the objectives for the plan of action must be focused on preventing or reducing injury and harm, and serious medical and psychiatric illnesses. Short-term goal for the tenants will include to avoid going out when is dark to prevent injury and harm. Provide education in the apartment to help the residents understand how to prevent injury related to the absence of streetlight. In addition, provide alternatives for individuals that are wheelchair bound to help them get around while there is construction going on in the neighborhood. For instance, provide education about meals on wheel for tenants that are wheelchair bound. Educate residents about other resources in the community that can help provide food on site. Help tenants contact local business owners that might be willing to come on
