Community Connections is a mental health agencies established in 1984. Its mission during establishment was to, “provide mental health services to consumers who were transitioning from institutional care into society” (“Community Connections History,” 2008). The focus of the agency then was to, “provide treatment for people who were diagnosed with major mental disorders and needed help to regain skills lost after years of institutional care” (“Community Connections History,” 2008). The founder Maxine Harris and co-founder Helen Bergman soon found out, a great number of consumers they desired to help had substance addictions along with their mental disorder. The journey for the founders was not one that did not experience great lost, in 2011 Community Connections co-founder Helen Bergman passed away. With continuous growth and development at every level the consumers’ needs have always remained the top priority and driven the many expansions of the clinical program. “Community Connections ' core purpose is to provide behavioral health, residential services, and primary health care coordination for marginalized and disenfranchised women, men and children living in the District of Columbia who are coping with mental illness, addiction, and the aftermath of trauma and abuse” (“Community Connections History,” 2008). When the client gives permission, the client must disclose in writing what they do and do not want to be disclosed and the disclosure is valid for 1 year. If a client does not give disclosure, disclosure can only be used to, “protect client or another individual from risk of imminent and serious physical injury” (“Authorization for Disclosure,” 2015). It can also only be used to “facilitate the delivery of mental health services of an individual in lawful custody” (“Authorization for Disclosure,” 2015). Clinical Practice Principles and Standards provide staff with methods when working with clients, methods such as; engaging, understanding, planning, implementing services, and achieving positive results. Engaging requires the worker to build a rapport with the client as a collaborative effort. Understanding is used to help the worker develop an “accurate assessment of the consumer’s immediate and long term needs, problems, and strengths” (“Clinical Practice Principles and Standards,” 2015). Planning requires the worker to assist the client create realistic goals and a plan to achieve them. Implementing services is the way the workers provide capable, appropriate, and essential and effective services.
