Community Connections Community health nurses form a crucial connection between the medical teams treating the patient and the community they live in by promoting preventive care, achievement of health and well-being, and patient empowerment (Balcazar et al., 2011, p. 2202). Community health nurses connect with partners in the community who can assist them in carrying out their goals. By partnering with doctor Clinton Sharp, a primary care physician, and Tommy Morris, owner and pharmacist of Trinity Specialty Pharmacy, I can assist in bridging the gap between diabetics in Slidell, Louisiana and the community resources that are available to assist them in achieving dietary compliance and medication adherence.
