Introduction
Hypertension, most commonly known as high blood pressure, is a serious cardiovascular disease that affects about 1 of 3 U.S. adults—or about 75 million people. ("High Blood Pressure (Hypertension) Information | cdc.gov," 2018). High blood pressure is a common condition in which the long-term force of the blood against your artery walls of the heart is high enough that it may eventually cause health problems. ("High blood pressure (hypertension) - Symptoms and causes," 2018). Risk factors associated with high blood pressure can be modifiable, such as diet, physical activity, tobacco use, and weight. The nurse’s role focuses on prevention, education, blood pressure screenings and monitoring, care coordination, and social barriers for individuals of the community in managing high blood pressure. Southeastern Health’s Community Health Services (CHS) department provides education, screenings and prevention services to the community through fire departments, churches and community programs. (Public Relations, 2017).
Community Assessment This community assessment project will focus on educating individuals with high blood pressure in Robeson County and on the importance of medication compliance, risk factors, lifestyle changes, and prevention to reduce comorbidities such as stroke, heart attack and renal disease. Education and materials will be provided at the Community Health Education Center located in Biggs Park Mall in Lumberton, NC. According to Robeson County’s most recent Community Health Needs Assessment report completed in 2014, 35.86% of the population reported hypertension as the number one concern reported to them by their doctor. (Robeson County Health Depart, 2014, p. 21). This community assessment project will focus on educating individuals with high blood pressure on the importance of medication compliance,

The county has a total area of 951 square miles (2,460 km2), of which 949 square miles (2,460 km2) is land and 1.8 square miles (4.7 km2) (0.2%) is water. It is the largest county in North Carolina by land area, but not by total area, that title going to Dare County. The county is surrounded by Bladen, Columbus, Cumberland, Hoke, and Scotland counties. ("Robeson County, North Carolina," 2018). According to the US census the Robeson County population in 2016 is 134,168 with 23,000 people living in Lumberton. ("About Us: Lumberton Visitors Bureau," n.d.). Robeson county’s population is comprised of: Native American-41%, Caucasian-25.5%, African American-24.2%, and Hispanic-8.6 %. Females make up 52% of the population and males 48%. Median Household Income is $31,298 the national household income is $55,322. ("Robeson County Demographics and Statistics,"
