Abstract
This paper discusses the Columbia shuttle crash disaster that occurred on February 1, 2003. Pressure suits, seat restraints, as well as the space shuttle crew’s helmets failed to work. The result was a devastating incident as the out-out-control ship broke apart after losing pressure. Following the incident, no survivors were rescued. A report from NASA claims that the incident led to the death of all astronauts that were on board. Serious criminal and ethical issues characterized the disaster. For instance, three members of the crew failed to wear gloves that would have protected them from depressurization. If not all, one of the astronauts failed to put on
