PROCEDURE: Colonoscopy.

INDICATION
Screening colonoscopy. No previous colonoscopy performed.

DESCRIPTION OF PROCEDURE
A timeout was called. Consent was signed. Perianal inspection normal. Rectal exam normal. Preparation was good. Tolerance was good. The forward-viewing colonoscope was advanced to the cecum. Photo documentation of the appendiceal orifice and ileocecal valve.

FINDINGS
Cecum and ascending colon: Normal mucosa. No polyps, diverticulosis or colitis.

Transverse and descending colon: Normal mucosa. No polyps, diverticulosis or colitis.

Rectum and sigmoid colon: In the proximal rectum, there was a flat 4 mm polyp, likely hyperplastic based on appearance this was removed with the biopsy forceps with
