Collaborating with the client is very important in the nurse – patient relationship as we need to work hand in hand to achieve the planned goals. I had this patient who had a Congestive Heart Failure, who was experiencing difficulty of breathing, with productive cough and was restless. As a nurse, following the nursing process, collaborating with the client at this point was addressing his complaints, attending his needs and concerns and for me to do that, I did an assessment for both subjective and objective data. Also, I gave my priority nursing intervention which was to position the client on bed comfortably in a fowler’s position and ensured that he was receiving an adequate amount of prescribed humidified oxygen. Afterwards, I informed my Clinical Instructor about it and of course the staff nurse on duty and verified my assessment. We notified the physician in charged and informed him about our assessment. The physician came and verify the assessment and we had discussion, explanation and exchange of thoughts and ideas about the case.
