Agenda – Team Meeting for Collaborative Care
In attendance: Team Leader – Registered nurse-she is accountable for patient related task. Nurse manager-to motivate and encourage nurse to implement delegating skills. UAP-to effectively communicate on skills of duty requiring his assistance. LPN- to effectively communicate reasons to increase tasks in order to provide high quality care for the patients as a team. Discussion Items:
1) Placing a white board in patient’s room so that patient, family members, and other team members can see daily agenda for care - i.e. physical therapy, medication needs, discharge schedule. This would assist in communicating with other team members and establishing a routine for the nurse to follow.
2) Daily briefing
