The only notable finding from the patient’s peripheral laboratory data (CBC, chemistry) is a creatinine of 2.1 mg/dL. What is the best treatment for this patient?
A. Colchicine
B. Indomethacin
C. Intra-articular methylprednisone
D. Morphine
E. Surgical irrigation and debridement

Answer
Choice “C” is the best answer. This patient has acute pseudogout which has the same treatment as acute gout. Colchicine and indomethacin are the mainstays of pharmacotherapy but are contraindicated in patients with renal disease. This patient has a creatinine of 2.1 mg/dL, which is correlated with an eGFR of 33 mL/min and represents stage 4 chronic renal failure. Both colchicine and indomethacin should be avoided. Intra-articular steroids can be safely used

What are some of your concerns based on the medication regimen the patient report? (4 pt) Some concerns based on the patient’s medication regimen would be how intense and sever her pain is. It is so severe that it is waking her up in the middle of the night making her take more medication. Juvenile gout is not a frequently heard diagnosis but it does exist and many times it gets confused with other conditions. Gout in children comes as a result of inherited conditions over producing uric acid such as Lesch - Nyhan Syndrome (LNS) and PRS superactivity. Lesch – Nyhan syndrome is an x-linked recessive disease and it is caused by a deficiency of the HPRT enzyme, causing a build- up of uric acid in all body fluids resulting in hyperuricemia (high level of uric acid in the blood) and hyperuricosuria (high level of uric acid in the urine) and potentially generating severe gout and kidney problems (Torres & Puig, 2007). The first symptom of this disease is the evidence of sand-like crystals of uric acid in the infant’s diaper. The over…
