On 08/27/2016 at approximately 8:00 PM, this Investigator conducted an interview with Cody Cummings (DOB: 06-18-02) at the DCS AHIT office (3221 N. 16th St. Phoenix, AZ). According to Cody, he was in a verbal argument with his mother approximately three weeks ago and was kicked out of his home by his mother as a result of the argument. Cody stated that the argument was over him acting out and not listening to what his mother tells him to do. Cody said that he had frequent verbal arguments with his mother by he has never done anything physical to her or anyone else in the household. Cody did state that his mother "shoved him in a closest" just before he was kicked out of the home. Cody also stated that he gets into frequent arguments

Yorkshire Dr. #1049, Phoenix, AZ. Present danger was found for Cody Cummings as Cody was kicked out of his mother's home and running away from his grandparent's home. According to Cody's mother, Cody threatened her and his siblings by saying he would stab them. Cody's mother gave power of attorney to Cody's grandparents as she stated she could no longer care for Cody. Cody's mother stated she will not allow Cody back into the home and is unwilling to parent Cody. Cody's grandparents stated that Cody is out of control and has severe behavior issues that they cannot handle. Cody's grandparents will not allow him back into their home and are unwilling to parent or care for Cody. No other family or friend placement is available for Cody. Cody ran away from his grandparent's home and was located by police and brought to DCS AHIT. Both Cody's mother and grandparents understand and approve of Cody being taken into DCS/State of Arizona custody. On 08/27/2016 at approximately 8:00 PM, this Investigator interviewed Cody Cummings (DOB: 06-18-02) at the AHIT DCS office located at 3221 N. 16th Street, Phoenix, AZ who appeared safe and healthy. However, Cody did not have a home to go to and is considered homeless. As such, Cody is not safe outside of DCS/State of Arizona custody. On 08/27/2016 at approximately 6:30 PM, this Investigator observed Devyn Cummings (7) at her home located at of 2702 W. Yorkshire Dr. #1049, Phoenix, AZ as being healthy, safe, clean, and in a safe and clean home environment. On 08/27/2016 at approximately 6:30 PM, this Investigator observed Hailey Cummings (8) at her home located at of 2702 W. Yorkshire Dr. #1049, Phoenix, AZ as being healthy, safe, clean, and in a safe and clean home environment.
