The CNL actively provides direct patient care in complex situations, serves as a lateral integrator who provides centralized care coordination for a distinct group of no more than 15 patients, and puts evidence-based care into practice to ensure that patients benefit from the latest innovations. A CNL is a leader in the health care delivery system with expertise in quality improvement and cost-effective resource utilization.
