CM-Toby completing discharge ISP for Julia. CM was unable to conduct a formal discharge meeting due to no contact with the Saginario family. Prior CM conducted a CFT back on 5/11/17 and since then CM-Toby have not been able to get in contact with the family since that date. CM-Toby made multiple attempts to call the family, left voice messages and no contact letters was even sent out to the Saginario family. CM-Toby have moved forward with discharging Julia from CMO services.

Julia was referred to FACT by Saint Clares Hospital CCIS Unit. Julia was admitted to CCIS due to suicidal ideation and thoughts of running away. Julia struggle with feeling pressure to find a job and school stressors. It was reported that youth struggles with high anxiety,
