Usually during the interprofessional patient rounds, all the members of the health care team work together for the benefit of the patient. But during the rounds, sometimes the health care team come across challenges which need to be encountered. For instance, antibiotic was prescribed to a 24-year-old patient (30 kg), but according to the patient’s weight the dose was very high. Intensivist and pharmacist recommended to decrease the dose or change the antibiotic based on the patient’s weight and health condition. The conflict was between the two doctors in the interprofessional team.…
Financial aspect: it will not cost anything to develop a unit-specific bedside reporting form and the actualization of performing the bedside reporting. My mentor will be assisting on coordinating with the clinical analyst and information technician department on proper installing the bedside reporting form as part of the daily assessment. We are currently using a software program designed according to the patient’s population being admitted to the hospital. The software program consists of basic nursing focused assessment. Therefore, the daily assessment tool bar will just need to be enhanced where there is an assigned tool bar system for bedside reporting to take placed.…
The class that was taught in the recorded lesson was a kindergarten class, with a class size of about 24 students. This lesson was the beginning of a new unit in chasing, fleeing, and dodging. In the prior units students learned about spatial awareness, levels, locomotor movement (running, jogging, walking, & different directions). During transition of care, patients often experience a change in their current medication regimen as well as be prescribed new medications. Often when a patient is admitted into a hospital during transition, their medical records are not easily accessible which leaves the attending physicians unaware of current medication regimen or allergies. For my first clinical rotation at Elderwood of Lake Placid for the spring semester, it was a good one. At Elderwood, we started the day at seven am with our morning debridement of where we’d be that day and the patient we would be advocating and aiding for. I had patients DM and PH. For the patient DM, I administered medication and because DM is one of the residents who does not need as much assistance, I spent most of the day getting to know the resident,which was a real treat because you can tell that many of the nurses and cna don’t really have enough time of the day to speak to each resident. Performing a change-of-shift report at the patient’s bedside is a best practice and insures quality hand-off. It increases patient satisfaction, promotes patient safety, gives patients opportunity to ask questions, correct any misconceptions and be more involved in their care. Present practice the nurses at this unit is not consistent with bedside reporting. Furthermore, patients are not included in the report.
