Clinical Practice Reflection

I set three specific goals for my first week back in clinical practice. I want to work on a few areas that I found challenging from last semester. My first goal was completing my head to toe assessment in a linear fashion, taking a break in the morning and completing my documentation in a timely manner allowing myself to have more time to learn about my clients’ condition, health history (by going through her chart and care plan), be on time with my charting and arrive on time for our clinical debrief.
My client was complex with CHF, hypokalemia, weakness, MRSA and was on contact precautions and had 11 medications to be given by 8am. Though not a terribly efficient process on my behalf, I felt working with my client enabled me a great deal

One area in particular that I reflect on was how being efficient with clients on contact precaution not only saves time, but decreases the risk of spreading the organism. There were a number of times that I felt I had everything I need and went in to my client’s room gowned and gloved only to realize I had forgotten and item I needed, thus causing myself to have to make more trips in and out of the room. This also caused more waste and laundry with frequent gown changes. When I reflect back to all the situations where I had not consolidated the items I needed for my client care and required more trips in and out of her room, I see that just taking a moment to breath and think about all the tasks and nursing interventions I will accomplish when I go to my client’s room. I look back on when I did forget items, I felt I was rushing too much and not critically thinking about my efficiency and reducing the amount of times going in and out. I was particularly conscious during the moments when she was not too motivated to do her own care. I remained conscious that I did not complete tasks such as ADLs that she was capable doing herself (need cuing), getting in and out of bed, washing in the shower and walking to and from the door. I wanted to enable her to do as much as possible herself and still remain respectful and not “tell her” how things were to be done. I always try to consider how I would want to be talked to and be empathetic to her health experience at the age of 87 years old. In talking with some of the other nurses about my client, so suggested I that I just “tell” her what is going to happen and not ask what she wants to do. I reflect on this in that there are was being kind and respectable in trying to motivate clients who are unmotivated.     I also learned how to combine and practice my assessments that I learned up to week 8. I was able to hear crackling sounds in the lungs of a resident who has COPD.
