The challenges that I face during my clinical health care setting relating to obtaining information is that a lot of the times I’m unable to find necessary information in client’s charts. Therefore, I would need to dig little bit deeper and search for needed information in other sources such as computer, notes, or asking nurse. Therefore, I believe that having all information in one place would benefit and have more time with our clients then searching for needed information.

At St. Anthony both electronic and paper charts are being used. I think that is it very important to use one source electronic or paper charts but not necessary both. It can get very complicated
