Consequently, in health care, the customer is the patient. Users of this tool do not have to be formal leaders within the organization, but in this phase at MHLB, we…
PCC can also improve clinical documentation by how they're charting templates that intuitively prompts staff to review and chart all pertinent clinical information (). PCC has a database that functionally allows users to enter information in one part of the chart and direct that information to other sections for auto-documentation as clinically indicated, such as the Resident Assessment Instrument/Minimum Data Set. This will ensure that every member of the healthcare team is being provided the most up-to-date clinical…

I participated in a discussion just a few days ago with a fellow nurse and a nurse anesthetist regarding respecting staff and ensuring patient safety. We were able to come up with an approach to heighten patient safety and recognize the value of everyone’s role in patient care. Another colleague asked for my assistance in developing a tool to aid nurses in planning for staffing. The tool we created will provide the documentation necessary for increased staffing on days when patient acuity is higher than normal.…
Clinical Writing Sample Initial Overview Raleigh Michelle Mallory is a 32 year-old, pregnant, twice-divorced, Native American female from Neah Bay, Washington. Ms. Mallory moved to Port Angeles, Washington when she was seven years-old, with her mother who was facing employment challenges on the Neah Bay Reservation. She vividly expressed during this recollection her past/present struggles with the dichotomous nature of tribal expectations and cultural integration associated with living outside of the reservation as a Native American on the Pacific Peninsula. In terms of family structure, Ms. Mallory is the second-child of a single-parent family. Her mother worked at a local grocery store for twenty-years before retiring, leaving her to oversee her sister to whom she…
Financial aspect: it will not cost anything to develop a unit-specific bedside reporting form and the actualization of performing the bedside reporting. My mentor will be assisting on coordinating with the clinical analyst and information technician department on proper installing the bedside reporting form as part of the daily assessment. We are currently using a software program designed according to the patient’s population being admitted to the hospital. The software program consists of basic nursing focused assessment. Therefore, the daily assessment tool bar will just need to be enhanced where there is an assigned tool bar system for bedside reporting to take placed.…
Throughout the entire semester, I incorporated evidence based practice into my clinical days. Whether if I was performing a skill or if I was observing my nurse do a skill, I was thinking is this evidence based. I observed nurses use water instead of alcohol to clean an area on the skin before pricking a patient’s finger for a blood glucose check; this I knew was evidenced based. Some none evidence based thing that I observed were using 5/8inch needles when the nurse should have been using a 1inch length or not using two hands when assessing a patient’s fundus. I met my midterm goal of telling the nurse nicely that this is not what we learned in school.…
Being at the Phoenix Center today made me see how people’s attitude can affect a person. Patient’s in detox centers such as Phoenix have set schedules to abide by as part of their treatment plan. It is nesseccary for them to attend to these private and group meeting to help them stay on track. Many of them see these meeting as pointless and stupid but they are very beneficial to their progress. These patients are also grogy and tired as they are coming off of drugs and alcohol, and the medications they take also make them tired as well.…
With technology we can see trends, lab work, and gain a better understanding of each patients needs. This can lead to better outcomes for all patients and even help patients be discharged from the hospital in a timely manor. As a nurse we look to do the right thing for all patients and this is a great tool that can aid us in…
I think most organizations use the dashboard because areas of opportunities can been seen easily, just like you mentioned. My clinical site also uses dashboard to display outcome measures. It seems that the CNLs at my clinical site and yours are very involved in monitoring health care outcomes. My preceptor and your preceptor can easily recognize potential opportunities for improvement. CNLs must have the knoledge and skills to be able to diagnose microsystem problems (McKeon, et al., 2009).…
Marissa and Amber both work at Aurora St. Luke’s Medical Center in Milwaukee, Wisconsin. They both work in the medical field, however Marissa is a Certified Nursing Assistant (C.N.A.) who is currently working full time and Amber is a Registered Nurse (R.N.) who is currently working part-time and attending school to get her Master’s Degree to become a Nurse Practitioner. Written communication is imperative in their workplace. Although they were both unaware of how often they actually use written communication, we were able to uncover many different ways they use it on a daily basis through an interview. Marissa, C.N.A. at Aurora St. Luke’s Medical Center:…
This will improve health outcomes and reduce duplication of nursing efforts. This will also alleviate the strain of not having nursing input when the EHR implementations are being done. Nurses are the central hub of information in healthcare. Nurses interact and interface with every aspect of communicating and assessing patient care needs. This is why successful EHR adaptation hinges on nursing leadership that understand the importance of having nursing input on the design, delivery, training, and implementation of an EHR system (Sensmeier,…
A patient navigator can be an asset to a health care clinic. Their job focusses on providing the ins and outs of the health care setting. They apply knowledge that helps patients to navigate the healthcare system and receive the timely care they require. When patient navigators started being utilized in clinics and hospitals, they were solely used for cancer care. Now that is has been shown that patient navigators are a vital part of the healthcare team, they are being used to help patients with other chronic diseases.…
