The ROD is a monthly review of the care given to residents in aged care facilities, it is documented in resident progress notes and provides a summary of the nursing care plan (NCP). In geriatric nursing, the NCP addresses and outlines the key areas of individualised care (Burhenn, Perrin and McCarthy, 2016). Currently, in my workplace, the ROD is handwritten by the registered nurse (RN) on morning shift and takes approximately 20 minutes to complete. I chose this audit as it is rarely passed with 100% compliance and it is a basic standard that should have passed easily. All ROD documentation will be dealt with systematically, with the titles already provided on a set ROD assessment form, which will decrease the time needed to complete the ROD and increase the reliability of the data with no area missed out on review, which is currently a pitfall. Change in resident needs can easily be identified looking at last month, with each section clearly labeled. Colour-coding will be introduced which will be beneficial for quick referencing of any multidisciplinary team documentation. Making these changes will provide a more efficient and effective documentation system that will ensure the future prospect of 100% audit
