CM normally meets with the client every Wednesday for face to face meeting on 10/5/2016. Client was no show. Client reported she had a doctor appointment scheduled on the same day. On 10/6/2016, client walk-ins Social Service Office and CM completed Bi-weekly ILP Review.
Client was well dressed and well-mannered as usual. Client continues to show the following sign: eccentric personality disorder, client continues to make allegation about some residents in her dorm taking snap pictures of her naked. Client also mention to CM that she have a Police Report against a formal residents that no longer in the shelter.

CM inquires the reason client was recently swap to another dorm. Client reported Administration swap her to another dorm because

Client is currently seeing by Ariel Nassin MD/Internal Medicine locate at 1000 Northern Blvd, Suite 375, Great Neck, NY 11021 tel# 516-773-3924. Client mentioned Dr. Nassin referred for Physical Therapy 3x a week at the same facility.

Client is undocumented and her housing package is incomplete. Missing the following: Green Card and income.

CM reiterate the shelter rules and regulation and for the client to adhere to the 10pm curfew.

Client continues to show with transferences she states that some residents and staff are taking nude pictures of her and then post it on the internet. Client is advise to attend to all Woodhull Mental Health Counseling for Individual Counseling.

Plan of action:
Client must participate in all scheduled meetings. Client must participate in free immigration service, client must comply with the shelter rule and regulation and 10pm curfew, and the client must provide staff with any changes of her status. Client must accept first suitable housing offers, client must seek alternative housing, and the client must attend to all on-site and off-site medical appointments.
