On 11/19/2016, CM met with the client to complete Other ILP Review. Client was dressed in appropriately for the weather. She was well-mannered and groomed as usual. She was cooperative, friendly and forthcoming with all information. Client continues to denied suicidal or homicidal ideation.

Today in the meeting client mentioned to CM that she spoke with son Dominican Republic who is requesting for the client to assist him financially. She also report family members here in NYC but she cannot reside with them because her biological parents suffers from Alzheimer's and they also reside in a Section 8 building.

Client continues report she is employed as a Line person in Perfume Factory in New Jersey; and sometimes due to low productivity she

Client also mentions she has an active Medicaid. She also submitted a copy of HRA letter dated 10-28-2016, inviting selected Cash Assistance recipients to a special pre-screening for Certified and Non-Certified Home Health Aides to work in all 5 borough and the pre-screening was scheduled 11-9-2016 @ 1pm at 123 William St, 6th Floor, New York, New York. Client continues to report she just received the letter yesterday but she is not interested.

She also received a Recertification Questionnaire/for Eligibility and CM assisted the client to fill out the form.

Client continues to reports chronic arthritis in her back, neck and leg, sleep apnea, hemorrhoid flare up and asthmas and receives medical services at Union Community Health Center.
Client reported no changes in medications, and she takes the following medications: Ibuprofen 600mg,
Ventolin HFA, Aspirin EC 81 mg, Simvastatin 20mg, Lisinopril 20mg, Naproxen 500mg, Calcium 600mg, Cetirizine HCL 10mg, Metoprolol tartrate 50mg, Aceta-gesi 12.5-325mg, Voltare (as directed) artificial saliva aerosol solution simvastatin 20mg and Advair HFA.
