Circumstance: Ms. Smalls (MHP) made face to face contact with Mr. and Mrs. Gailliard (MHS) and Clarence.
Action: Clarence was hiding the TV when MHP enter the home. Clarence seem excited by making loud noises and shaking his head back and forth. MHP and MHS discuss social interactions with peers his age and younger. MHS explain Clarence will talk and play with the peers for a short time than separate himself. Clarence answered questions when asked. MHS report Clarence struggle with following directions and working on hygiene. MHS explain Clarence is not honest all the time and provide examples. MHP provide MHS encouragement to think outside the box when providing Clarence interventions.
Plan: MHP will ensure Clarence well-being with weekly
