Due to the trouble of categorizing a patient with chronic pain, I would complete comprehensive health history assessment. This will provide subjective and objective data from the patient along with the examination of the patient. This history will provide knowledge of the current events or activities causing the pain (Jarvis, 2012). The second health assessment, I would perform is a detail pain assessment. I would use the initial pain assessment tool as detailed in the book. The initial pain assessment tool will be of effective use in the chronic pain patient. HPI: Patient reports continuing back pain, Ibuprofen 800 mg taken twice this morning with no relief
PHI: Chronic Illnesses- Diabetes, Hypertension,
Treatments-none, Surgeries- appendectomy (Feb 2000)
Active/Chronic Problems- back pain Allergies- Sulfa medicines
Substance –drinks on occasions
Exercise- works out 30 min in gyms 3x week,
Nutrition –regular diet, healthy, Sleep Pattern-8-10hrs nightly
Medication History- Current Medications –Losartan 50/12.5mg 1 tab daily, Ibuprofen 800 mg tid for back pain, Norvasc 10 daily
Health Maintenance-Immunization: Flu shot Feb 2014, physicals-last Feb 2014 Occupation- works with computers, Nature of work-36 hrs per week
Work setting- works on feet at a Best Buy, work related symptoms: back pain ibuprofen

Family History- father passed from cardiac arrest, mother still alive, Diseases- paternal grandfather diabetes, and hypertension
Psychosocial and Spiritual- Divorced, Christian, No Children, Church weekly, little stress
