CHIEF COMPLAINT
Knee pain.

SUBJECTIVE
Patient is here for re-evaluation of chronic right knee pain. He has had an ACL repair on two occasions. At his last appointment, he was prescribed meloxicam and referred to physical therapy. He reports the meloxicam is not any more helpful than over-the-counter NSAIDs. He has not been seen by physical therapy yet. He is requesting a cortisone injection. He has had an injection after his first surgery and previous to his second and did get some relief from it.

OBJECTIVE
Generally, well-appearing male. Ambulates with an upright, coordinated gait. Vital signs are reviewed as well as the x-ray of his knee.

ASSESSMENT
Chronic knee pain.

PLAN
Because he has had several surgeries and there is hardware
