Chronic conditions and diseases such as cancer, heart disease, obesity, stroke, arthritis and diabetes are among the most common, preventable but costly of all health problems. Modern medicine has been criticized to be focusing more on identifying and treating acute problems resulting in fewer resources being available to chronic illness, which is equally important. This has been the trend despite the ever increasing numbers of chronically ill patients. The growing need for better chronic illness care has increased the urgency for health care delivery systems to resolve the issue of many deficiencies in the management of chronic diseases. Therefore, this paper will discuss the resources on chronic care and chronic illnesses currently available to patients, families, and communities.
Centers for Disease Control and Prevention The Centers for Disease Control and Prevention (CDC) is one of the divisions dedicated to preventing, raising awareness and controlling chronic conditions and diseases. It has set up the National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) essentially for this purpose. Through the NCCDPHP, it has developed quite a number of resources and tools aimed at helping the policymakers, public health professionals and the community in their efforts of preventing chronic diseases.
