This model offers a framework of six elements that should be considered during chronic care. The first element is having an organization, mechanism, and staff that promote safe and high quality care. The second element, delivery system design, suggest that the patient receives care and support in the best way for the patient. Self-Management support is the third element; this prepares patients to provide their own healthcare. A strategy for this could be helping the patient realize what they need to work on and help them set reasonable goals they can achieve at home. The next element is decision support; this emphasizes the importance of using your multidisciplinary team to help make the best decisions for your patients. The fifth element is clinical information systems. A strategy for using this is looking at prior documentation in the electronic records to see how your patient’s health is trending. This could show you what is staying steady and what needs to be improved and talked about with the patient. Lastly we have the community element. There are several different strategies a nurse could use for this element; the nurse could give the patient information about local support groups, exercise groups, educational programs or anything in the community that would benefit the patient.
