A review of her medical records indicates that she suffers from chronic back pain due to spinal stenosis and was being managed by outpatient pain management. She was hospitalized in December with complaint of headache, generalized weakness and falling at home. She suffers from chronic COPD and is currently titrating down on her prednisone, HTN which is stable at this time, GERD-stable and carotid stenosis which is stable.
A today’s visit, she is home alone. She is awake, alert and oriented. She reports chronic back pain that has worsened. She describes her pain as dull with a severity of 8/10. She states that her pain is worse with movements. She reports that she is on a pain regimen of oxycodone/apap 7.5 mg p.o twice daily as needed for pain,
