Samuel (Sam) is under the Special Education team (Alison Jones, Early Intervention) who provides therapy and support regarding his needs and a learning plan. Sam attended the Child Development Team (CDT) with his Dad Lyle this morning. Please see Dr Grangaard’s report (dated 23.03.15) further background information.
Developmental Profile:
Sam was assessed using a developmental screening assessment - the Schedule of Growing Skills (SOGS). This explores areas of language, fine and gross motor skills and allows the therapist to observe Sam’s abilities. The assessment was completed at CDT. He is now going on the toilet to urinate, preferring to pass bowel movement in a nappy and then show this in his hand, to inform of occurrence. Sam is receiving a nappy subsidy and on review Lyle confirmed today that this supply is suitable, therefore will be reviewed in six months time. He continued to explain that Sam will wash and dry his hands, and attempts to brush his teeth. I have provided Lyle with a ‘lift the lip’ pack (children’s tooth brush and paste) for Sam to promote the practice and independence in brushing teeth activities. Sam may benefit from watching ‘Tom and his toileting triumphs’ on you tube or DVD to provide insight on the toileting process. The DVD can be loaned from CDT if required.
Interactive social skills: This looks at how Sam copes with separation, independence, play and self image.
Lyle highlighted that Sam has friends that he likes to play with. Sam appeared to enjoy ball games, playing skillfully and was able to throw the ball with his right hand overarm and kick the ball using his right foot.
Gross Motor: This looks at co-ordination, balance and large motor skills.
Sam happily stepped up and down stairs in the gym play area, two feet to a step, right foot leading ascending and left foot first descending and using the wall for support. Sam was not willing to hop on one
