ONU decided to implement a new funded program into their health system, Health-Wise, which consisted of four pharmacists, one nurse, two exercise physiologists, and a nutrition coach. In the past, Health-Wise had an older program that had a great success in assisting patients with managing chronic diseases, which happened to be only a selected portion of the university’s population. Therefore, it was the university’s decision to incorporate a new program that provided assistance to healthier members who may not have qualified for the disease management program. The new program was designed to focus on patients with chronic diseases as well as preventing disease in healthier individuals. Not only did the pharmacists use internal and external resources to help them with their newly implemented program, but also due to the success of the first 9 patients, preventive health screening was incorporated into the routine practice of the clinic. This comes to show that the healthcare team worked together to achieve favorable results. On the downside, collaborative practice could have been used even more effectively if the understanding of the university’s budget and cost savings was applied. According to paper article, “A determination of specific cost savings would be beneficial in determining the exact worth of pharmacist involvement in this area, because it is often difficult to determine the cost savings associated with disease prevention, especially in the short-term.” In other words, some chronic diseases are usually expensive to treat, which can lead to pricey treatment, prevention and education, especially in the long-term. Therefore, in the future, the program would have to gather data regarding the rate at which patients choose to receive follow-up from outside healthcare
