Interdisciplinary meeting about this case took place on 6/28/17. The areas identified for improvement are:
• Implementation of wound care plan at Bascom Palmer.
• Discharge plan starts as soon as the admission is ordered; effective communication between the attending physician and nursing is essential to coordinate the best care for the patient.
• The discharge plan needs to be reviewed every day to evaluate the appropriateness of care and modify any intervention base on the patient needs.
• Nursing needs to be proactive in identifying patient needs, and quick and effective interventions should be implemented for the benefit of the patient.
• Appropriate escalation of complications following the Chain of Command.
