1. Cervical polyp. A cervical polyp is a tear-shaped or lobular structures that appears red, purple, or flesh-colored located on the cervix that is usually smaller than three centimeters (Laufer, 2016). Chronic inflammation of the cervical canal and hormone factors may increase the rise for polyps to develop. Often times the cause is unknown. A patient with a cervical polyp will present with painless spotting that occurs after sexual intercourse or irregularly during the menstrual cycle. A diagnose is made when visualizing the polyp during a pelvic exam or with pelvic and transvaginal ultrasound if cervix if poorly visualized. Treatment for cervical polyps is to do a polypectomy. This patient presented with painless irregular vaginal spotting between her menstrual cycle. When performing a pelvic exam, a 2cm polyp was visualized at 3 o-clock on the cervix. This is the final diagnosis for this patient. 2. Pregnancy. Pregnancy is when an offspring is developing inside a women’s uterus. The patient may report irregular spotting during the first trimester. Chronic endometritis. Chronic endometritis is an inflammation of the inner lining of the uterus. According to Kaunitz (2016), approximately 15% of women with chronic endometritis present with intermenstrual bleeding and postcoital bleeding. The diagnosis of chronic endometritis is made clinically based upon symptoms and can be confirmed with an endometrial sample. A patient with this condition often present with vague lower pelvic pain and cervical motion tenderness. However, some patients may have a completely normal examination and without a fever. This patient denies lower uterine discomfort and does not have cervical motion tenderness. She does not have a history of sexual transmitted diseases and her LPS was normal. This diagnosis is unlikely because other than spotting irregularly during cycle and after intercourse, her symptoms are not consistent with this
